It's easy to give feedback
Here is our handout that explains how to give feedback and outlines what we will do once we have received your feedback.
- Complete the online feedback form below; it will be acknowledged by a member of our staff. Please allow 5 working days to receive an initial response.
- Alternatively, after your appointment, you can speak with the Practice Manager or receptionist about your experience before you leave. If they are not readily available, you can ask for a Feedback form to fill out and hand in before you leave.
We want you to know
- We take your privacy seriously, and will ask your permission to share information with the relevant staff member/s to resolve any issue that has arisen.
- Our feedback process follows guidelines set by the Health and Disability Commission, which promotes and protects the rights of patients. www.hdc.org.nz
