The new My Health Portal<\/strong> (my indici\u2122) sign up page will require four pieces of information – your:<\/p>\n

  1. First and last names.<\/li>\n
  2. Date of birth.<\/li>\n
  3. NHI (not mandatory)<\/li>\n
  4. Email (needs to match the email we have on file)<\/li>\n<\/ol>\n

    My Health Portal <\/strong>(my indici\u2122) will match all the information entered in your registration with your patient record. If this matches, you will have Level One access to My Health Portal:<\/p>\n

    Level One Access<\/strong>: Provides ability to make appointments and email our practice.<\/p>\n

    Level Two Access<\/strong>: Requires PFHC to verify your identity in order to grant you full access to your records. To do this you can phone or ask reception next time you visit the practice.<\/p>\n
